Important Information for Patients

We can all help control the virus if we all stay alert. This means you must:

• only meet in groups of up to six people in any setting (indoors or outdoors) and you must socially distance from people you do not live with
• wear a face-covering where required, unless you are exempt
• wash your hands frequently
• provide full and accurate details of everyone in your group when visiting a café, pub or restaurant as part of the NHS Test and Trace Service
• stick to the full guidance around car sharing
• self-isolate immediately if symptoms appear and book a free test by calling 119 or visiting the NHS COVID-19 tests webpage.

The Practice will close from 12pm on certain third Wednesdays. Dates will be advertised on the Practice notice board. This is time for staff training and development.
There is no surgery on a Saturday morning. Please use the Out of Hours number, which is for emergencies only. The number is NHS 111

The Practice is now working in collaboration with other local practices, Churchside Medical Practice, Roundwood Surgery, Millview Surgery and Forest Medical, to offer weekday appointments out of our core hours from 6.30pm – 8pm and Saturday mornings 8am – 12. These are provided on a rota basis at each practice as per the list here

The Practice offers SystmOnLine to patients who are registered at the Practice.
This means that you can book appointments, cancel appointments, request repeat medication online.

If you wish to use this system, please call into the Practice and speak to the reception team who will be able to set this up for you and supply your log-in information.

GDPR
The new General Data Protection Regulations (GDPR) came into force on the 25th May 2018 and the Practice is committed to being compliant with the new regulations. Information regarding how this affects the patients can be found here

The Practice will not discuss any information with family members unless we have explicit consent from the patient or the relevant Power of Attorney information
